• A completed application form for the Czech long-term visa (the red form) .
• A international passport ,that is going to be valid at least 15 months from the application date.
• A Birth certificate
• A contract with the Center regarding to education service
• Confirmation about the exception to the course (it is given by the Center)
• A contract with the Center about the finding the residing place
• A confirmation to the Center about the residing place in Prague (it is given by the Center)
• A bank statement confirming that the applicant (or one of his parents) has the account in any currency the rest on which makes not less an equivalent 3500 Euros
• A International Credit card (for example Visa, MasterCard, Maestro...) addressed to the applicant, given out to above specified account
• A certificate of criminal record from the citizenship country (if any)
• An Insurance policy issued in one of the Czech insurance companies, with the insurance sum covering not less than 30000 Euros and valid all term of the future visa
• 3 color photos 3,5 х 4,5 sm.
• The health condition inquiry(for citizens of Ukraine)
• About 150 Euros for consular fee

In a case the applicant is under age: the notary consent of both parents to departure to Czech republic for educational purpose
Rules of the visa reception
• The filing of the application and documents is made preliminary by the telephone to a consulate (embassy) employee, thus concrete date and time, or via the Internet - through system VIZAPOINT (www.visapoint.eu) will be offered to the applicant.
• The filing of the application and documents is carried out personally by the applicant, as well as receiving the visa.
• The consideration of the application and visa registration can take up to 60 days from the date of applying.
• The final decision about visa issue is accepted exclusively by consulate employees and the police of foreign affairs . In some cases the consulate reserves the right to require the applicant for providing with extra documents, to invite to additional interview.
